Preparing for Your First Appointment

When it comes to your cancer treatment, you have a choice. Thank you for choosing the Ella Milbank Foshay Cancer Center for your treatment; it is our privilege to care for you.

How to Prepare
Before you arrive for your first visit you will have received a packet of information either through email or postal mail from our front office team explaining where to go and what to bring. Please read this information carefully.

Before Your Appointment

Insurance
Our insurance verification specialist will contact your carrier prior to your first appointment to determine your plans benefits and coverage for your treatment. Additional calls may be made to your carrier after your treatment plan is determined. Our insurance verification specialist will discuss your plans benefits and any out of pocket expense you may incur.

Medical Records
Most patients are surprised by the amount of preparation needed for the first appointment. Cancer is a complex illness, and each case is different, it is important for us to have as much information about your case as possible.
You may be asked to bring pathology slides, medical records or imaging studies to your first appointment. In some cases you will be asked to provide that information prior to your first appointment.

Medical History & Medications
Prior to your first appointment you will receive a packet of information including a medication list and current and past medical history form. Please complete these and bring them with you to your first appointment. Be sure and bring your insurance cards with you as well.

Maps & Directions
The Foshay Cancer Center is located on the campus of Jupiter Medical Center with convenient parking on the north side of the building. If you are unfamiliar with our location please review the maps & directions section of this website so you can become familiar with our location.
